A gunsmith will just get some brass rod a little smaller then the bore diametre and knock it out with a hammer.

Out of interest, do you think it is better to try to get it out of the breech or muzzle end? I would have thought it better to try to continue to move it down the barrel and remove it from the muzzle end, but that's just guesswork based on the idea of trying not to fold the cloth back on itself. Would be good to have a more informed opinion!

If the patch was on a wrapped jag it might have come off on the reverse stroke which would very much influence the approach to extraction as WG alludes to.

My bet however is the patch is not far up the bore as it got stuck a few inches after passing the chamber neck and when trying to reverse it came off??

This has happened to me,just fill up the barrel with wd40,leave for a wee while to soak and it'll come out easily with your rod.
