Electronic ICR's

Has anyone got electronic ICR's (word document) that they can send me please? Hoping to be an AW and think these would be extremely helpful as I write like a 5 year old using crayons.

I did used to have a copy of them but that was a couple of years ago and I have a new computer as my last one died, never to be "Har Drive DataCopied".

I havent got any either and to be honest my writing is blood awful. We were told that we could copy out the ICRs and then type in the answers and staple them into the port folio. However if they are already about it would save a huge amount of work.


Yes please if anyone has got a set, I would be very greatful.

The following are the blank templates that I used for my own Level 2 Portfolio. Apart from changing both the Canditate's and AW's Number's and Name's, the ICR number and page numbers can also be amended. Once completed and attached to the Portfolio I also signed and dated all of the pages like the Candidate's Additional Notes etc.

Deer Management Qualifications (DMQ) would like to thank “Roe Hunter” and “Mr Lewis” for posting documents on this site to help other DMQ Approved Witnesses. It is encouraging that witnesses are willing to help one another in this way.

In consideration of the demand, DMQ has now produced editable pdf templates for the Witness Record of Questioning, Candidate Narrative and Witness Additional Notes; they are available on the DMQ website.


Evidence compiled by Approved Witnesses (AWs) and Candidates using these templates will then have to be printed and inserted into the relevant places in the portfolio; all such inserts must be signed by the Witness and/or Candidate, as appropriate. The blank, non–editable, ICR pages already on the website can be used as before to produce paper inserts.

DMQ is not currently prepared to accept any of the portfolio sheets in electronic form, the portfolio must be presented in hard (paper) copy. Stick in labels will continue to be acceptable for AW contact details after element 4 of each ICR, and for contact details on the administration details page.

To maintain authenticity and control of submitted material, from 1 August 2012 DMQ will only accept paper inserts of portfolio forms which are produced using the DMQ templates; no alternatives, nor any electronic submissions will be accepted. The usual checks that the material submitted is authentic, current, sufficient and relevant will continue to be made.

All AW’s will be e-mailed advising them of the above.

For those with Adobe Reader, the new templates can be completed electronically and printed for insertion into the relevant page of a DSC2 portfolio. Using Adobe Reader, changes to the form cannot be saved and if witnesses wish to maintain a copy for future reference a second copy will need to be printed. If Adobe Acrobat or Adobe Creator is available, electronic copies can be saved to file but completed forms must not be shared for use by other Witnesses or Candidates.

Is the new PDF ready to be used if so would you like the word doc attached to this thread removing?

It is indeed - you can find them here: http://www.dmq.org.uk/download.htm

The downside, I believe, is that if you only have Adobe Reader on your computer then you can't save any changes made, so you will need to complete the full document and print it out (twice if you want to keep a copy yourself), rather than being able to save changes part way through and then go back and complete it later. Still, full marks to DMQ for making these available.
