CV for Level 2 portfolio

Im just starting my Level 2 and Im after a template or an example of the CV that should go into the portfolio. I know it need to be my CV however i would like to see what sort of things should be included.

Two suggestions:

1. Contact Mandy at DMQ for guidance http://www.dmq.org.uk/contact.htm
2. Draft your CV in Word or similar. Include any experience relevant to DSC II and run it past your Approved Witness for his comments.
