With regards to a single shot causing hearing damage I have just experienced this and had the effects recorded.

I was shooting my 375 with a muzzle brake and had not correctly fitted my ear plug for just one shot.

My hearing test 24 hours later showed that I had reduced my mid range hearing by 26 decibels. My next hearing test 2 weeks later showed that my hearing had recovered slightly with only 10 decibel reduction measured from my previous hearing test last year.

I wear hearing aids as a result of shooting, chainsaws etc.
