You need to put your name and address on the form before giving it to the doctor. Otherwise how will he know who's form it is?My coterminous certificate is due for renewal my doctor is okay with filling out the relevant section.
What I want to know is do I give my doctor the form after I have completed it or do I give it to the doctor completely blank.

You don't fill in anything after your signature space.
You send every page (5 pages in total I think) and the GP fills in the rest.
If you get it pack you only need to send the pages the GP filled in to the FLD.
SeptemberAs M@tt has said.. however, you’ll need the returned GP Proma / scanned or e-mailed electronically BEFORE you start the on-line renewal process… You can’t stop and save half-way through.. as you’ll need to upload this at the SAME TIME you complete the application / process, ensuring it’s well ahead of the minimum 8 week statutory period, else you’ll be forced to store you firearms with an RFD..
When does you current certificate expire?
Plenty of time then, my experience and checking by the FLD published timescales, they were recommending submitting 6 months prior to expiry.. so you’ve a month or two yet..September
