You need to put your name and address on the form before giving it to the doctor. Otherwise how will he know who's form it is?

The medical report form your doctor needs to fill in is a separate form from your FAC application form. Give your doctor the blank medical report form with your name and address added and get him or her to send it back to you, electronically (pdf) via email if you are doing an online FAC renewal, then you can just upload the completed medical form.
Are you getting confused about the two forms? There is no reference to the firearms you possess on the form you give to the doctor.

Note that when i renewed last year my doctors surgery came back and ask me for £150 fee for filling it out. I used MediCert at half the price in the end.

I know charges from GPs for this are all other the place. I am fortunate to live in the sticks in Herefordshire where almost everyone seems to own either a shotgun or rifle so my local village GP is used to doing it. I get the completed form emailed back as a pdf to upload with my application in about three days for a cost of £36. I don’t take for granted how lucky I am to get this level of service.

The medical pro forma has the first two pages as a letter from you to your GP. You fill this in and sign.
You don't fill in anything after your signature space.
You send every page (5 pages in total I think) and the GP fills in the rest.
If you get it pack you only need to send the pages the GP filled in to the FLD.

As M@tt has said.. however, you’ll need the returned GP Proma / scanned or e-mailed electronically BEFORE you start the on-line renewal process… You can’t stop and save half-way through.. as you’ll need to upload this at the SAME TIME you complete the application / process, ensuring it’s well ahead of the minimum 8 week statutory period, else you’ll be forced to store you firearms with an RFD..

Plenty of time then, my experience and checking by the FLD published timescales, they were recommending submitting 6 months prior to expiry.. so you’ve a month or two yet..
I submitted mine in September 2024 for and end of December 2024 expiry, was granted a Temp Certificate after the 8 week extension expired, then certificate actually landed some time in April 2025.
