My SGC is up for renewal in the summer.

Under the new rules do you need to supply a complete medical history or just changes since the last renewal or my FAC grant about 18 months ago? TIA

Download the form from your police licensing department. Ensure that the Dr fills in all the boxes. Don’t over complicate things worrying about what needs doing.

It's the same form, get the doc to fill it in fully and completely, they have a habit of missing the odd bit and getting it returned.

You’ll need a doctor to complete a new medical proforma along with your SGC application form.
