Errors on medical records

My FAC/SGC are up for renewal at the moment.

I got all the renewal documentation in before all the current covid panic began, and well within the 8weeks that my force require.
Had a call from the FEO a few days ago, to do a telephone interview (given the current situation).
The renewal should have been straight forward, as nothing has changed here and nothing to declare on the medical side. But I was asked a few times about drinking and alcohol, which seemed odd.
I was told that my GP had been contacted as that is D&C policy now, and that they had alread replied to the police request. (I've not been contacted for a fee yet, so will see if one arrives.)
However, there was something on my record which was concerning, hence the questions. The issue was quite old, so was not necessarily a show stopper. But a problem that I'd never declared it previously. I was quite shocked when I was told what it was, as it was definitly an error. The FEO was very understanding and contacted the doctors again to have it checked. Which the practice did immediatly and confirmed the error. Apparently the practice were very appologetic!
So everything is going through without issue now, but I still feel a bit uneasy about it.

I mentioned it to a friend who is a medical doctor, and was tild that errors on medical records are pretty common.
In my case, I have been with the same practice for 20yrs+ and the documentation about the incident was still on file, even though it was 13yrs ago.. If this had not been the case, I could have had quite a battle to get it corrected.

Hats off to the practice for dealing with the police request at this time, and sorting the problem. Even though they made the error in the first place.
But, once the current situation is over, I am going to request a copy of my full records, and check them through. Not only for FAC reasons, but I don't want to be ill in future and have doctors making decisions based on wrong info.

On the up side, I am getting an open ticket. And another 'few' slots to fill.

It might be that the policy is as previously in place with D&C and they are still complying with the existing HOG - ‘tick box’ letter to GP and proceed with issue even if no reply received.

As that part of the process has apparently been already satisfied in your application, I’d think that you can safely ignore any invoice from the surgery (if one is even issued).

I went through my own renewal with D&C last October and even then they were conducting interviews by phone and no mention of deviation from HOG over medical evidence. Overall I’ve found the D&C FLD to be generally very efficient and pragmatic.

You’re close to me, don’t suppose your registered with CMP at Purewell cross? I renewed back in Dec and was never contacted by surgery for any costs, but did notice when I had an appt that they had put two markers on file with “SGC holder” and “FAC Holder”. D&C were good to deal with from my experience, good FEO who was very helpful.
 
The tick box form for my GP to fill in, was indeed done. But the problem was, that there was a very serious medical condition/intervention that was reported, which I should have declared on my renewal (and the two/three previous renewals).
But this was something that never occured. It turned out that someone else's details had been put on my medical record, in error. And has sat there for 13 years, only coming to light now.

Once I assured the FEO this I had absolutly no knowledge of this, they went back to the doctor and the issue was resolved.

My concern was, that if there had not still been the paper trail to follow, to show up the error. I could have had a long battle with the medical world. And whilst this was going on, probably had my FAC revoked and the possability of being charged with making multiple false statements on my renewals.

But telling the story here, in case anyone else falls foul of errors on medical records.

I posted a related experience in February following my annual check up with the practice nurse. The check up went very well until I checked my records on line afterwards. She had recorded a coded entry regarding my mental health as ‘depressed mood’. I contacted the practice and spoke to the nurse who stated that it was ‘a mistake’ and that she ‘couldn't understand why she had done it’. She then changed the entry to read ‘normal’.
 
Sounds like it is worth it for anyone who has not yet been through the doctor's letter process yet, to check through their records and make sure they are accurate.
 
When I requested a copy of my records from my local surgery for a matter not connected to firearms i had notes saying i had an ovarian cyst removed!!!! If my doctor thinks i am female I would not like to see his wife.
