I have a similar problem with my lab and am looking for an answer. Mine has red patches under his ears too and shakes his head and scratches his ears. Also licks and chews his feet a lot.

Looks like ear mites, you can get drops from vets or I just use olive oil, a few drops in each ear and massage ear canal.

Olive oil , get it to the Vet diagnosis is the first port of call, you may just stop a case of vestibular disease which is not nice to see ,dogs don't make a fantastic recovery from it ,the vet is the man the can check for ear mites otodectes not something you can see with the mark one eye ball microscope job .
