guessing but you probably cannot find a form as it’s not part of the home office approved form, so just include her details on a separate sheet of paper, together with her birth certificate, marriage certificate, job details, bank accounts, shoe size, favourite handbag make etc ………… etc…..

Its standard practice.. email them and ask them to send it.
Its for background checks that's all.

If its the same as Northumbria it asks who else lives in the house with you.
Relationship to you and asks for DOB.
Also interestingly it states does not need to be filled in but may slow the process if not or words to that effect.

As a matter or interest, how did your licensing department react to that? Because background checks on other members of the household is in the Statutory Guidance…

The on-line process doesn't give you an option to submit the form.

I received an e-mail from the administration staff to acknowledge my application and which invited me to return the form that had been attached to the e-mail 'to prevent delays in processing my application.' I thanked her for the e-mail and highlighted that by their own admission on their website, submission of the form was not a mandatory requirement, to which her reply was: 'Whilst it is not mandatory to complete the form, we are required to complete vetting on the occupants of the address. When the form is not completed the firearms enquiry officer will request this information at your home visit.' I thanked her for confirming and highlighted that we looked forward to the officer's visit. I didn't hear anything further about it, since in the event, the FEO wasn't able to attend my home due to traffic delays and he processed my renewal over the phone whilst he was driving. I received my co-terminus certs about a week later.

Quite how taking the names of two people during the home visit can delay the application process is beyond me. It's another example of mission creep on their behalf and an infringement of freedoms. I've held a certificate since I was about 15, I'm now 57. I must be one of the lowest risks on their books.

I see what you mean about the extra form being another thing, but having had to fill one out myself it does seem like the simplest way to do things.
